Forklift and Order Picker Course Assessment Information

Assessment facts

After you have completed your forklift and order picker training with The Operator School, you must successfully complete 3 units of assessment to obtain the paperwork necessary to facilitate the issuance of your forklift licence (order picker licence) in Queensland.

  • Theory
  • Calculation
  • Practical

*The theory and calculation tests must be successfully completed BEFORE the practical assessment can be undertaken.

Standardised assessment for safety and quality assurance purposes

The forklift and order picker operator assessment process is similar in every Australian state and territory.

All components of the assessment are mandatory and supplied by Safe Work Australia.

The Assessor on duty is required to select a number of questions from within the framework of the exam. As a result, there will always be variations between each assessment.

Under Workplace Health and Safety Legislation, Assessors are required to submit details of all upcoming assessments to Workplace Health and Safety Queensland’s head office in advance.

Inspectors from Workplace Health and Safety Queensland in Brisbane have the authority to conduct an unannounced audit of our Assessors, the trainee and the forklift and order picker licence training facility at any time.

Theory Assessment Facts

  • The exam must be conducted in English therefore you must be able speak and understand English well (No interpreter is permitted.)
  • It is not multiple choice but a series of short statements or one word answers
  • It is a closed book assessment, i.e. you cannot take any learning materials or notes in with you
  • It is permissible to undertake the theory exam orally for students who struggle with written English

Forklift

Test is made up of 58 Questions
* The exam should not take any more than 2 hours to complete
Critical answers 28 You cannot get any of them wrong.   Any wrong critical answers result in immediate fail.
Non Critical answers 30 You need to get 90% correct to achieve competency i.e. allowed 3 wrong answers before a fail is recorded

Order Picker

Test is made up of 50 Questions
* The exam should not take any more than 2 hours to complete
Critical answers 25 You cannot get any of them wrong. Any wrong critical answers result in immediate fail.
Non Critical answers 25 You need to get 90% correct to achieve competency i.e. allowed 2 wrong answers before a fail is recorded

What happens if you get any of the questions wrong

  • Any critical questions answered incorrectly will result in you having to re-sit the written exam again
  • Any non critical questions that were answered incorrectly will be discussed with you. These questions will be asked again at the time of the practical assessment to confirm that you now have the knowledge.
  • If you got more than the permissible amount of non critical questions wrong you will have to sit the whole written exam again.
  • You can still carry on with your training if you fail your assessment, you will just re-sit the assessment at a later date before your practical assessment.

Calculation Assessment Facts

  • Oral test or interpreters cannot be used
  • It is a written exam
  • It is a closed book exam – no learning material with you
  • You are allowed a basic calculator only
  • You must show the formulas (working out) for all your answers
Forklift 8 Questions All Critical answers – cannot get any wrong
Order Picker 5 Questions All Critical answers – cannot get any wrong

* The exam should not take any more than 1 hour to complete

What happens if you get any of the questions wrong

  • You can still carry on with your training if you fail your assessment, you will just need to re-sit the assessment at a later date before your practical assessment. (3 day notification maybe needed)

Practical Assessment Facts

  • You need to achieve a successful outcome in the theory and calculation assessment before a practical assessment can be undertaken
  • Your practical assessment will be conducted in the English language (No interpreters)
  • You will be asked questions at various points during your practical exam (when safe to do so) to confirm your knowledge of the task at hand. Your answers to these questions are taken into consideration as part of your whole assessment; they cannot solely make you competent or not competent.
  • If you got any non critical questions wrong during your written assessment you will be asked these questions at some point during your practical assessment, these answers too have to be correct.

Test is made up of 5 tasks
* The exam should not take any more than 2 hours to complete, however it should be done in a similar timeline that it would normally take in a workplace
* You need to get 100% correct to achieve competency

Task 1 – Hazard Identification and Risk Control
Task 2 – Pre-Start Safety Check
Task 3 – Start Up Safety Checks (machine running)
Task 4 – Operate machine to the full range of its capacity (safe movement of loads)
Task 5 – Shut down and secure the machine

  • Evaluation of the worksite
  • Identify any hazards within the work area
  • Plan work, make sure machine capable of doing the job you are required to do
  • Select (or be wearing) correct Personal Protective Equipment i.e. safety boots (or enclosed shoes at a minimum and a high visibility shirt plus any other item necessary to that worksite i.e. hard hat if a construction site
  • The student must conduct a detailed pre operation check
  • Shift 3 different types of loads (minimum of 250kg each)
    Lift one – low level
    Lift two – medium level (not less than 25% of the machine’s height capacity)
    Lift three – high level (not less than 75% of the machine’s height capacity)
  • Negotiate obstacles (chicanes)
  • Demonstrate good safe operations at all times
  • Correct post operational checks and shut down procedures

Practical Assessment at your work site

If you want Practical Assessment to be conducted at your work site you must meet following requirements

  • A WHS Inspector maybe there at the time of assessment to audit the Trainee, the Employer and the Assessor
  • A suitable area to perform practical assessment
  • Forklift is in good working order i.e. data plate, seatbelt, beepers, manual, that meets AS 2359
  • 3 loads (not less than 250kg each)
  • Barriers, signs or other items to create control measures for pedestrian zones
  • Need high, mid and low racking that complies with Australian Standards
  • Aware assessment may affect production
  • Any on site requirements, site induction, blue card that our Assessor needs to comply with?
  • Somewhere to do necessary paperwork
  • A practical assessment should not take any longer than 1 hour

Re-Assessment Facts

If you fail part of your assessment, i.e. you have booked for your theory and calculations, you pass the theory and fail the calculations, the following process will apply:-

Within 2 calendar months of failing the first time round
The trainee need only successfully complete the calculations and the practical assessment to be deemed competent
The competent result in the theory assessment will still be taken into consideration

More than 2 months elapsed
The trainee must complete the entire assessment again (theory, calculations and practical)

More than 12 months elapsed
The trainee must start the whole training course again from scratch.
